| All employees of the TDSB, except those hired after March 31, 2006, are required to complete the
on-line 2006 Offence Declaration Form. Employees hired on or after April 1, 2006 will be required
to complete this on-line form starting in June of 2007 for the 2007-2008 school year.
Failure to complete the Annual Offence Declaration Form is a breach of the legislative requirement for continued employment with a school board. The name of any employee who fails to provide the required document will be forwarded to the Executive Superintendent of Employee Services.
To access the online TDSB Offense Declaration Form from any computer outside the TDSB network, you will need your Employee number and a network password (not TEL id and password) to login.
If you do not know your network password or your password has expired within the 42 days, please call the Enterprise Help Desk at 416-395-HELP (4357) Option 5 during office hours, 7:30 a.m. to 5:00 p.m., Monday to Friday.
If you have any convictions since September 2005, please have all related documents with you before you begin the Offence Declaration process. To start your Offence Declaration, enter your Employee Number and Password and click on the Login button.
